Dear Sir,
I am writing in response to your request for additional information for block number 3 of the accident reporting form. I put "poor planning" as the cause of my accident. You said in your letter that I should explain more fully and I trust the following detail will be sufficient.
I am an amateur radio operator and on the day of the accident, I was working alone on the top section of my new 80 foot tower. When I had completed my work, I discovered that I had, over the course of several trips up the tower, brought up about 300 pounds of tools and spare hardware. Rather than carry the now un-needed tools and material down by hand, I decided to lower the items down in a small barrel by using a pulley, which fortunately was attached to the gin pole at the top of the tower.

So, he climbed out of the hole and walked over to his boss. "Why are we digging in the sun and you're standing in the shade?" he asked.
"It's called 'intelligence'," the boss replied.
"What do you mean, ‘intelligence'?" asked the man.
The boss said, "I'll show you. I'll put my hand on this tree and I want you to hit it with your fist as hard as you can." A small company was on the brink of bankruptcy, so the owner came up with a sales incentive program to help motivate his salesmen.
Calling his two-man sales force into his office, he announced, "Things aren't looking good and unless we increase our sales, we won't be in operation much longer. In light of this, I have come up with a a contest for you. The guy with the highest sales will receive a blow job."
"What does the loser get?" asked one of the salesmen.
"The loser," replied the owner, glaring at both men, "gets to give it!"

According to the FBI, most modern-day bank robberies are "unsophisticated and unprofessional crimes," committed by young male repeat offenders who apparently don't know the first thing about their business. This information was included in an interesting, amusing article titles "How Not to Rob a Bank," by Tim Clark, which appeared in the 1987 edition of The Old Farmers Almanac.
Clark reported that in spite of the widespread use of surveillance cameras, 76 percent of bank robbers use no disguise, 86 percent never study the bank before robbing it, and 95 percent make no long-range plans for concealing the loot. Thus, he offered this advice to would-be bank robbers, along with examples of what can happen if the rules aren't followed:

A building contractor, working on a job that would take several months, was being paid weekly. Looking at the check he had just received, he approached the owner of the property.
"Excuse me, but this check is two hundred and fifty dollars less than we had agreed upon," he said.
"Yes, I know," said the owner. "Last week I overpaid you by two hundred and fifty dollars and you never complained."
"I don't mind an occasional mistake," replied the contractor, "but when it starts to be a habit I feel I have to bring it to your attention."
